well@home simplifies your life. You will quickly come to view well@home as a personal health assistant. The information collected by well@home can be made available to family and loved ones. Loved ones also have access to the education and instruction available from well@home. St. Joseph’s/Candler Home Healthcare manages the well@home program in your area.

Do you need help following your doctor’s orders?

well@home will:

• Educate you about your health, your doctor’s orders, and your medications.
• Help you take measurements, such as your blood pressure.
• Remind you of activities you need to perform, such as taking a medication.
• Alert you and St. Joseph’s/Candler Home Healthcare if there is any concern about the health information it collects.

well@home will also:

• Help you perform your doctor’s orders.
• Fit into your daily routine.
• Enable you to easily manage your medications.
• Ensure that St. Joseph’s/Candler Home Healthcare has all of your latest health information.
• Promote a healthier, more independent lifestyle.

well@home is not an emergency response system. In the event of an emergency, you need to call 911. Ask your doctor about well@home and whether this device is right for you.
