The Place at Pooler (TPAP) is a 122-bed skilled nursing facility serving the needs of Pooler and surrounding areas. Our community offers convenient access from Bloomingdale, Rincon, Savannah, Garden City, Port Wentworth, and Springfield via interstates 16 and 95.

The professional staff at The Place at Pooler delivers long- and short-term care in a warm personalized manner. Our mission is to provide a flexible, interdisciplinary approach to health care centered around caregiver compassion and delivered in a comfortable, home-like setting. We serve the healthcare needs of patients through a full spectrum of extended care services. Our team of healthcare professionals tailors programs to meet each patient’s specific needs. In this way, we ensure the best possible care for every patient.

Our Continuum of Healthcare Services

The Place at Pooler serves patient needs through multiple levels of care, including skilled and traditional care, as as well as hospice care.

Skilled Care

The Place at Pooler meets the special needs of skilled level patients with Physical, Occupational, Speech, and Respiratory Therapies, Central Lines, and IVs combined with 24-hour nursing care. Whether provided for several weeks or several months, our physician-directed healthcare services are designed to complement a hospital stay, or in many cases, be a substitute for hospitalization.

Traditional Care

The Center provides consistent nursing care to traditional care residents for support in their daily activities. With our care and support, which can range in duration from several months to several years, these patients are able to achieve and maintain the highest possible level of function.

Rehabilitation Services

The Center’s comprehensive rehabilitation services round out our spectrum of care. These services, provided by our in-house licensed rehabilitation personnel, are available on an inpatient basis and serve those who wish to return home. Licensed Physical, Occupational, and Speech Therapists are available to offer services for most rehabilitative needs. The Place at Pooler has helped many residents regain their health and be discharged within weeks of their admission. Our community has recently expanded its Rehabilitation Department to include household appliances, to provide more realistic rehabilitation for those who wish to return home.

Activities and Stimulation

Patients at The Place at Pooler enjoy a dynamic Activity Program that offers regular stimulation with up to five activities per day. Various games, music, and parties are a part of each day. Volunteers also provide spiritual support through religious services, and they help out with daily activities. Additional personalized activities are offered as necessary.

24-Hour Security

Designed for residents who are at risk for wandering, The Place at Pooler offers a 24-hour security system. If a resident attempts to exit an exterior door, an alarm will sound to alert our staff.

Payment and Support

The Place at Pooler accepts Medicare, Medicaid and Private Funds. Medicare, Medicaid, and private insurance have benefits that cover nursing home stays. Private insurance may require prior approval. Assistance is available in applying for any of these programs. Private payments are also accepted. A resident may remain in our community as long as there is a medical need. Should a resident wish to return home, as he or she is able, outpatient therapy services are available through Medicare Parts A and B to provide support during the transition to life at home. Also, our Social Services Department is available to assist with discharge planning.
